Auth Cancel not appending ?error_reason=user_denied was marked as a duplicate of this bug.

We have an app which uses FB authentication on server side. When a user clicks on the App URL he is presented with the Auth Dialog box but if he clicks cancel, he will be kept on showing the same dialog box rather then taken to code to handle the exception. This was working fine till last week.

Steps to Reproduce:

1. Hit the URL http://apps.facebook.com/mdhilreader/ on your browser.
2. You will be given the FB Auth Dialog box.
3. Click on Cancel button.
4. There will be a redirect to http://apps.facebook.com/mdhilreader/?fb_source=dialog_permission and not to http://apps.facebook.com/mdhilreader/authenticated/?error_reason=user_denied&error=access_denied&error_description=The+user+denied+your+request.&_=_ .
5. The Dialog box keep on coming.

Expected Behavior:

The URL should be redirected to http://apps.facebook.com/mdhilreader/authenticated/?error_reason=user_denied&error=access_denied&error_description=The+user+denied+your+request.&_=_ so that we can handle it on our server end.

Actual Behavior:

The Dialog box keep on appearing even after clicking on cancel.
